North Shore Lactation services are fully covered by most major health insurance plans. Please find your medical insurer below to learn more about your likely plan coverage, but we always recommend you reach out directly to your insurer to confirm coverage and benefits.

If your medical insurance plan does not cover your visit, you can pay for your appointment (including through most HSA/FSA plans) and upon request North Shore Lactation will provide you with an itemized superbill receipt that you may submit to your insurer and request reimbursement. The policies and rules vary by insurer and are frequently changing, so we cannot guarantee that you will be reimbursed.

  • North Shore Lactation is an in-network provider with Aetna. Aetna will likely tell its insureds that they are covered for 6 visits but in reality, we are seeing that Aetna covers 3 visits, after which it assigns "patient responsibility" for the services provided for which the patient will be invoiced.

    If you have Aetna Better Health of Illinois (Medicaid plan), please ensure you have your OB/midwife complete this authorization form and date it on or before your appointment date: https://www.aetnabetterhealth.com/content/dam/aetna/medicaid/illinois/providers/pdf/Prior%20Authorization%20Form%20IL.pdf Failure to do so will result in Aetna not covering your visit and you will be invoiced the self-pay fee.     If you have Aetna Advocate Preferred, North Shore Lactation is NOT in network.

    If you have First Health, North Shore Lactation is NOT in network

  • North Shore Lactation is an in-network provider with United and UMR (except Advocate Aurora Preferred plans - contact North Shore Lactation to discuss). Most United and UMR plans plans cover unlimited visits at 100% coverage.

  • Through a partnership with The Lactation Network, those with BCBS PPO insurance plans are typically covered for unlimited visits at 100% coverage. Some of these plans are not included. To confirm your coverage through The Lactation Network, please complete this form. The Lactation Network will then email you (typically within 1 business day) to confirm your coverage.

    North Shore Lactation is an in-network provider with NorthShore 447 BCBS HMO plans, which provides 100% coverage.

    If you have any other BCBS HMO plan (such as Advocate), your insurance plan is out of network, but you can proceed with booking a Self-Pay visit. Upon request, North Shore Lactation will provide you with a superbill that you may submit to your insurer and request reimbursement. The policies and rules vary by plan and are frequently changing, so we cannot guarantee that you will be reimbursed.

  • North Shore Lactation is an in-network provider for Cigna but please call Cigna to verify your coverage. Some plans require a Network Adequacy Provision form (NAP) to be completed first.

  • Through a partnership with The Lactation Network, sowe patients with Anthem PPO insurance plans are typically covered for unlimited visits at 100% coverage. To confirm your coverage through The Lactation Network, please complete this form. The Lactation Network will then email you (typically within 1 business day) to confirm your coverage. If your plan is not approved, you will be able to self pay and submit a super bill receipt to Anthem for possible reimbursement.

  • If your provider is not listed above, please call or text 847-220-4192 to discuss your coverage. If your plan does not cover your visit, you may still book a consultation as a self-pay patient. You will be required to pay for the appointment at the time of booking. You can pay with credit card, debit card or through most HSA/FSA programs:

    • Initial consultations are $250

    • Follow-up consultations are $200

    If your medical insurance plan does not cover your visit, upon request North Shore Lactation will provide you with a SuperBill that you may submit to your insurer and request reimbursement.
